is your char human? if so i recommend picking up the least dragonmark of the sentinel. it gives you +2 intimidate and 3 shield of faith casts a day. the shield of faith can be dispelled, yes, but for quests where that doesn't happen (read as "in most quests") you can save an equipment slot that way as you don't need a protection item. it also opens up a an enhancment for +1 intimidate for 1 action point. so if you don't want to switch out anything else then throw out skill focus: intimidate. what that nets you is 3 shield of faith casts for 1 AP.

if you already got combat expertise i'd also take improved trip. a mob that is sitting on it's ass doesn't hurt anyone either. and it works also, or rather especially, with caster mobs where your AC is useless.
for the same reason i'd consider stunning blow.
you could throw out one or the other toughness or the greater weapon spec/focus for those 2 feats.

and you might consider bullheaded, it's +2 intimi and +1 will save. will save is more important than reflex for you. so you can switch it out for lightning reflexes. another save feat that's better than lighning reflexes is luck of heroes (+1 to all saves)

As I said in game, I'd consider getting khopesh and swapping out some of the shield related feats and some of the Toughnesses, while making sure you have more than 500 hp at cap at the least, including a +45 hp Shroud item and a Greater False Life item.

I've never played a ful-fledged tank and can't speak from my own experience, but you could have a look at this build, a European player who's been tanking since game launch and definitely knows what he's talking about. I'm not suggesting you should copy that build of course. It might give you some ideas though.
